Non-Cornell Users/No NetID - Complete an Occupational Medicine Questionnaire
Complete a health questionnaire if you have no Cornell NetID

If you have scheduled an appointment using myCority, you will receive an email asking you to complete a health questionnaire.
Filling out a Questionnaire will save you time during your appointment and ensure your health care provider has the information needed to provide proper treatment.
- Use the link in the email you received to log in to myCority.
- Select Applicant/Contractor/Non-Employee.
- Select Next.
- Enter your Temp ID from the email you received.
- Enter your Last Name and First Name.
- Select Next.
- Answer the questions.
- Select Submit at the top of the page to finalize your responses. You can also select Save if you need to stop and finish later, or Cancel if you do not want to submit the questionnaire.
- For Submit, you will see a confirmation Success – Record submitted.
- For Save, you will see a confirmation Success – Record saved.
- For Cancel, Cority will ask you if you want to leave the page:
- If you select Leave your unsaved changes will be lost.
- If you select Stay you have the option to Save or Submit your questionnaire.
- Log out of myCority or close your browser to protect your personal information.
