There are two central service offerings that can be used for surveys: Surveys for Research (Qualtrics) and Surveys Lite (Microsoft Forms).

Surveys for Research (Qualtrics) is a full-featured survey research tool that includes support from Qualtrics and the IT Service Desk. It is approved for use with human research when approved by Cornell's Institutional Review Board (IRB) under the terms of Cornell policy. 

Surveys Lite (Microsoft Forms) is a lightweight app for simple surveys that works directly in Microsoft Teams and saves responses to Excel Online automatically it can help you collect simple feedback, poll, and gather other form data.
